Bash Registration Form

This is a two part form, the top is registration for the Bash and the bottom is for the Fund Raising Fishing Tournament. Please fill out those fields that apply to you. You don't need to have a boat or a team to participate in our fishing tournament. Just let us know you are coming and who else is coming with you. Everyone is welcome!

The registration fee for the Bash is $20.00 per adult and $10.00 per child between the ages 6-12. Children under 6 are free. The registration fee includes dinner Friday and Saturday and eligibility to win a Big Kahuna award.
